Rural Care

In Manitoba, the province’s Community Paramedic Program is scaling up to serve more rural and remote communities. The initiative empowers paramedics to deliver in-home care to patients with chronic or acute needs—offering services like IV therapy, wound care, and post-hospital monitoring.

Health officials say it’s a game-changer for regions where hospitals are far and family doctors are few. The program helps reduce emergency department strain while keeping care personalized and close to home.

With the province’s support, the expansion reflects a broader commitment to decentralized, proactive care. Local leaders hope this model will improve outcomes and reduce unnecessary hospital transports.

Bonus Bite

More states are eyeing Mobile Integrated Healthcare to curb long-term Medicaid spending by avoiding costly ED visits and repeat hospitalizations. But consistent reimbursement and program sustainability remain sticking points.

Julota's MIH-CP software empowers community paramedics to deliver smarter, more connected care by simplifying fragmented data and streamlining processes. With real-time patient insights, automated reporting, customizable workflows, secure HIPAA and CFR-42-compliant collaboration, and actionable analytics, Julota enables impactful care and improved outcomes. Designed to bridge healthcare and social determinants of health, it helps your program stay ahead of change.

A New Study

A recent study published by Healio highlights how Mobile Integrated Health models are especially beneficial for women and younger adults recovering from heart failure. These groups saw meaningful reductions in readmission rates when they received in-home follow-up care from MIH teams.

Researchers noted that MIH visits allowed for earlier intervention and more consistent patient engagement—particularly helpful for patients who often feel overlooked in traditional models. The study suggests MIH can bridge equity gaps and improve outcomes where they’re needed most.

As programs expand, this kind of targeted success could guide how follow-up protocols are designed in the future.

Mobile Clinic

The University of Calgary, in collaboration with The Alex Community Health Centre and TELUS Health, has launched a Diabetes Mobile Clinic (DMC) to serve Calgary's underserved populations. This two-year pilot program offers on-site diabetes screening and assessments for complications like retinopathy and neuropathy, along with access to endocrinologists and diabetes educators.

The DMC employs the SAFER model—Screening for glycemia, Foot and Eye health, and Renal function—to provide comprehensive care. By bringing services directly to communities, the initiative aims to eliminate barriers to care and reduce hospitalizations due to diabetes complications. Dr. David Campbell, leading the project, emphasizes the importance of accessible care for those facing homelessness or other challenges.
