Region of the Week: Seminole County, Florida

The Seminole County Fire Department (Florida) won "Program of the Year" at a national conference in Chicago, beating out over 80 other submissions. Their Community Paramedicine Program, launched in 2022, sends paramedics on home visits to residents who struggle with healthcare access, frequently call 911, or are at risk of being readmitted to the hospital. Visits can include medication education, wellness checks, fall prevention, and connecting people with the right resources.
In 2025 alone, the program completed nearly 2,500 home visits, served 250+ residents, and hit a 90%+ enrollment rate with a 78% graduation rate. The goal is to catch problems early, before they become emergencies, easing the burden on both residents and the healthcare system.

New Funding
Governor Patrick Morrisey announced a new $1 million funding opportunity for EMS agencies in West Virginia to expand community paramedicine programs, with a focus on improving healthcare access in rural and underserved parts of the state. The initiative is part of the state's broader Rural Health Transformation Program.
The funding is designed to support models that allow paramedics to do more than just emergency response, including on-scene assessment and treatment, care navigation, and coordination with primary care, behavioral health, and social service providers. The goal is to reduce unnecessary ambulance trips and ER visits while connecting patients to better follow-up care.
Initial rollout will focus on a specific service region to test and refine the approach before scaling statewide. EMS agencies interested in applying can do so through wvOASIS.gov. The initiative is fully funded through the Centers for Medicare and Medicaid Services (CMS).
